Dilated and effaced: These terms refer to your cervix. 3 cm dilated means that the circumference of your cervical opening is 3 cm. Effacement refers to how flat your cervix is. A cervix can go to 10 cm or "fully dilated" and 100% effaced or "fully flattened".
