Intermittent brown spotting between periods mostly mid-cycle. transvginal ultrasound showed small fibroid. smear normal. why is it happening?

Spotting: A small fibroid is unlikely to cause bleeding unless it is "submucous", near the endometrium. However, causes of spotting are often hormonal, and further testing and perhaps oral contraceptives may be indicated.
