Yes: Absolutely, at the very least you should be on aspirin. I am surprised you are not getting any blood thinner medicine like warfarin after your stroke/cerebral infarction. You have anti phospholipid antibody syndrome-will suggest you to see hematologist. Also, . With positive ana, aca-igm and back pain, as well as stroke-you need to see a rheumatologist to eval autoimmune problem like sle.

Not sure: Exactly what you have. An aneurysm or have you had a stroke(infarction )? ANA antibodies for what? Do you mean anca? Sorry need more information or could really give you a wrong answer.
