Get evaluated: Cancer would be the first thing to rule out, especially if you have risk factors. There are several other causes of blood in sputum.

Serious symptom...: The most likely reason to cough blood is bronchitis but this symptom always requires an evaluation by a doctor because it could be due to malignancy. So, if you cough up blood, see your doctor asap.

Depend how much: If its a little bit in your phlegm then i would not worry about it. However if you are coughing up pure blood, then that is a sign of a serious disease. Many things can cause it, but most of them are bad. Tb, serious fungal infections, cancer, pulmonary embolism, arterial-venus malformation are a few. If you are coughing.
