Blood clots?: Unusual presentation especially without microscopic hematuria being present. Concerned about a potential foreign body that he may have placed transurethral leading to the marked reactive inflammatory cells on cytology. Could have him void using a strainer and see if he could bring in what he has been seeing. Maybe consider a renal bladder ultrasound.

Casts?: a urine microscopic examination is important here, Casts could be "worm like" particles. In an earlier era, urine specimens left at room temperature for several hour sometimes developed "vinegar eels" This is the result of improper specimen handling and hopefully is not the explanation for you findings. I would ask for a simple Urinalsis with microscopic examination
