Go to the ER now: The risk of dehydration is substantial here if all you're dealing with is gastroenteritis. However, given the child's age, I am most concerned about foreign body ingestion with impaction in the esophagus or even toxic ingestion (a 4 year old can take a swig of under-the-sink chemicals ; you may not have seen it). Don't take a chance--get the child to the er now.

Frequent small sips: Of clear liquids such as pedialyte or gatorade, as little as 5 -10 ml , every 10 minutes, may be tried. If tolerated well for at least an hour, gradual increase of pedialyte followed by introduction of light foods such as cereal or cracker will be the next step. If this measure fails, see his primary care provider.
