I have sudden pain top of right breast bone.pain is constant, also feels tender to touch and worsens when i extend my arm forward should i be concerned?

Be concerned: There may be a pulled nerve in your neck causing a "peripheral neuropathy" that worsens when you extend your arm forward. See a neurologist. An MRI might be ordered to find the nerve that is being pulled or compressed.
