Shin pain/fever: Need more info to even suggest a diagnosis. Children of this age may develop unilateral shin pain and fever if they have acute osteomyelitis, and there are other causes. Bilateral shin pain needs an exam to see if there are any skin changes and the cause of the cough. Tb can do this, along with numerous other illnesses. Have him seen by a pediatrician and good luck.

Hard to know: If he is not feeling well, unable to walk, and has fever, he should see a doctor. There are too many possibilities to try to guess what the problem may be, so he should be checked.
