Rare: Very rate, but can happen. In experienced hands and not emergency case it is very unlikely.

Not common: If the surgery is scheduled in advance ("elective surgery") so that a patient has been able to not eat anything for 8 hours, vomiting during induction of anesthesia is unlikely. And if it occurs, there should be very little volume of vomit and no food pieces, which makes it much less dangerous. However, there are medical conditions and medications that make the risk higher.

Is a risk: Vomiting and aspiration is a risk during induction of general anesthesia. This is why it is recommended that the patient had nothing to eat or drink for at least 8 hours before surgery. Also, there are medications that can be given to help supress vomiting.
