Abd ultrasound: This is both to evaluate the kidneys and determine if there is association of what can be seen in ultrasound and the bump in cr. Futhermore, right kidney is still part of the RUQ except posteriorly, and also you can see the RUQ with the ultrasound. If negative, then reconsider other modalities, work from it.

H;P, UA, US: You should see a physician. A good history and physical to assess volume status (among other things), a urinalysis, possibly followed by an ultrasound to look for obstruction.

Urine: A urine analysis would be the first step, if a Up/Uc if there is protein, a renal US and then medications are often culprit. Any recent cardiac intervention and or change in meds? is the BP too low for this person?
