Uncommon: Docs occasionally write the wrong dose or write with bad handwriting so that the item is misread. Pharmacists can pick the wrong item off the shelf or mislabel the drugs. These are becoming less frequent in this age of electronic e - prescribing. All in all mistakes in this arena are very uncommon.
