Portal hypertension: Alcohol, and other things (hep b/c and other diseases) scar the liver over time to the point where the main blood vessel that supplies the liver (the portal vein) does not have enough pressure to get the blood in, and the blood pressure builds up in this very large vein. This backs blood up into alternative pathways, including varicose veins of esophagus. These can bleed badly;mean cirrhosis.
