Hard to say: There are several possibilities, either it is too early to see anything and that conceived later than you thought, or that the pregnancy is doing poorly. You doctor will probably want to repeat the ultrasound to see.

Hard to say: Without being there or seeing the scan. If you had a vaginal ultrasound it would be quite unusual to not see a fetal pole at 7+ weeks. With an abdominal ultrasound (thorough the belly) i'm not surprised at all: they arent terribly good for first trimester. Check with your OB and good luck.
