Many ailments: Not knowing anything more, like his age, other medical conditions, and so on, it's very hard to say. He may have a simple bronchitis, or something more serious. You should have him see his family physician soon to follow up with a thorough evaluation.

Stiff neck: In the absence of a neck injury history a stiff neck may mean just a sprain on one end and meningitis and hemorrhage in the brain cavity at the other. You need to get him to see a doctor to find out the cause for the cough as well as the stiff neck. It is better to be prudent than sorry.

Yes: I would take him to the hospital. Fever, confusion, blurred vision, vomiting- point to something going on in his central nervous system and would need to be addressed immediately. This includes stiff neck.
