Pneumonia: If he indeed had pneumonia, he should have been treated with antibiotics, oxygen, respiratory medications to improve his oxygen delivery.

What was refused: If they refused treatments for the pnuemonia, not much else to add. Having said that, many stroke patients develop pnuemonia because they often times are at high risk of aspiration.
