TMJ Surgery: TMJoint surgery should should only be considered after all other conservative treatment by well known and recognized experienced TMJ specialists has been attempted and failed. They would be the most knowledgeable as to who would be best to perform surgery if that was necessary. Having said that, if your local DDS can't recommend someone, ask at local Dental School affiliated teaching Hospital

Last resort: Many oral surgeons will not perform TMJ surgery because of the inherent risks and proximity of the nerves, blood vessels, etc. Unless it is trauma related. Surgery of the jaw joint/joints should be done as a last resort and i would get more than one opinion. Most TMJ surgeons are associated with hospitals.

Regional Expert: The surgeon should be in your area. Too hard for follow up care from a long distance. Best bet is to see someone on the oral surgery faculty of the closest dental school. In the hands of a competent oral surgeon, TMJ surgery has a good success rate.
