With use of the vaccine there is marked reduction in the incidence of chronic hep b carriage which is generally how perinatal transfer occurs. In taiwan, for example, the rate has fallen dramatically. In third world countries it remains inordinately high since access to the vaccine or acceptability by the population is reduced.
