Depends if you mean: Pain with the original insult verse pain after stabilization and chronically. Its extremely rare to have a spinal injury without pain sensation, and that usually only occurs when it is a polytrauma situation and there is too much pain everywhere to recognize it. Chronically, you do not always have to have pain. Some injuries people can, if they are lucky, live pain free.
