Not all: There are pneumothoraces that are treated conservatively by observation or by oxygen for resorption of the no2. If large enough, enlarging or symptomatic, then thoracotomy is used to decrease the pressure on the hole to allow healing and avoid the complications of increased tension. The tube allows equalization of pressure in the thorax as in the lungsand the airways to allow breathing.
