Depends: This greatly depends on the situation and indication for the procedure. In general, you do the less invasive procedure whenever feasible; however, if the lesion is too large, if the hospital doesn't have the appropriate facilities, or if the surgeon is not comfortable with the technique, they will default to the older thoracotomy. Either: Acute hemothorax may be solved with a chest tube alone. If not then video assisted vats may suffice. If it persists and is thick and not draining, then either vats may allow adequate visualization and suction drainage of the clot.
