Minor differences: They both block aldosterone. Eplerenone is more expensive but it doesn't cause testosterone suppression. Spironolactone block aldosterone and testosterone so there is more side effects with this med. Pt on high dose spironolactone (more than 150 mg) has higher chance of developing gynecomastia. Due to cost, Aldactone is normally tried first, if there is side effect, then Eplerenone is used.
