CHF: From "tachy/irregular", i infer that he has atrial fibrillation with rapid ventricular response? I presume he has pulmonary edema as well as peripheral edema? I'm speculating since I have no data, of course. His doctors will slow his heart which improves pump function and then he'll be able to wean from the ventilator. He sounds volume overloaded? Hopefully his kidneys work?

Cor pulmonale: He might have mostly right heart failure, . For edema consider ultrfiltration. If not successful possible right or lrft ventricular assist devices.
