Only as needed: They should only be removed if they are broken or there is decay under the fillings they are inert and are more of a hazard removing. If they are large old fillings and you want white, you can have them crowned with porcelain. A cerec dr can do it same day.

Depends: The mercury used in dental fillings are not high enough to cause harm. If there is a cavity beneath the mercury filling it may need to be replaced. However the best person to ask is your dentist.
