Probably not heart: The fact that bending forward makes it worse makes me think it is most likely structural that causes the pain, which in turn increases your anxiety. My best guess without examining you and asking a few more questions is that the shortness of breath is related to anxiety (hyperventilation) from stopping smoking etc.. The shortness of breadth and the discomfort do warrant a formal evaluation.
