Sudden sharp heart pain occasionally (few secs) for years. got ekg; dr said showed irregular heartbeats but "nothing big". what could it be/next step?

Costochondritis.: Sharp piercing pain in anterior chest for years in young male is very unlikely to be cardiac disease unless there are other significant symptoms, signs detected on exam or a family history of sudden cardiac death. Pain is very likely due to muscles and nerves in the chest. If pains are frequent enough or triggered by activity, could consider avoidance or treatment with anti inflammatories.
