Ask your own doctor: First of all, you should be asking this question to the doctors who actually know you medically. If you have an infection, one would generally hold the Methotrexate for a week. However, this decision should be made by the doctor treating your lupus.

Yes: Stop the Methotrexate and push fluids. If you do not feel better in 48 hr see your rheumatologist for exam and lab work.
