It Can: The eustachian tubes open on each side above and behind the soft palate. If there is liquid covering the "cup" where the eustachian tube opens, then yes, the fluid can run down the tube towards the back of the ear drum.

Doubtful: It would be highly unlikely. But the nasal spray is designed to be used when you are in an upright position and thus i would not recommend using it lying down since it will likely be less effective.
