Can high blood pressure medication that dilate veins ever give the visual impression of varicose veins when there is no reflux present?

No: Most BP medications do not have significant effect on veins. If varicose veins are seen they are most likely there due to underlying venous disease.

Doubtful.: Dilated veins are not varicose veins. Varicose veins are grape-like in appearance and due to malfunctioning (refluxing) of the saphenous vein valves. Blood pressure medication will not cause varicose veins but it could cause dilation of normal veins and further swelling of already existing varicose veins but it will not cause normal veins to become varicose veins.
