Office Policy: Unsure of the office policy. You did miss the appointment, and that time was reserved for you.. If you like the office and don't want to look for a new dentist or possibly be sent to a collection agency and have your credit score tarnished, i would pay. Each office is different, and I do not know they will proceed on this matter.

History of : Do you have a history of several missed appointments? If so then $20 seems more than fair. If it is your first missed appointment and you are an otherwise responsible patient who always proceeds with recommended treatment, then call the office and see if they will remove the charge for this time only, but if they do, please do not miss any other appointments.
