Clot: When hemorrhoids get hard it usually is because of a thrombosis or clot of those veins. This will typically cause some pain. One approach is to have this drained, generally a simple office procedure. You should consult your primary care physician for initial evaluation and guidance.

Hemorrhoid is a vein: Hemorrhoids are veins and they can be either internal or external. External hemorrhoids can get irritated and the blood in the vein can clot. This is painful and gets hard and is called a thrombosed hemorrhoid.

Thrombosed hemorrhoi: Hemorrhoids can thrombose and such thrombosis can be painful, especially if they are external. i.e. external hemorrhoids. Internal hemorrhoids are rarely painful unless they are thrombosed.
