Rare but can happen : Usually development of plaque which is the culprit in causing heart attacks starts to develop at very young age but does not become a problem until middle age. People can have heart attacks at a young age ( less than 35) usually due to other causes aside from plaque. Short answer: can occur but extremely rare at this younger age group.

Would be rare: Having a heart attack at 28 would be very rare but could be a sign that other blood relatives (siblings, children, even parents) are at similar risk. This is usually due to extremely high cholesterol/lipid levels that run in the family, and cause plaque to develop in the blood vessels at a very young age. These people should be treated with cholesterol-lowering medications.
