No: The parenchyma of the lung has no nerve endings and doesn't cause pain. However, there is something called "chronic pericarditis" and (i'm not an expert at lung disease): probably chronic pleurisy too. Chronic pericarditis can cause on-going pain. If other causes are excluded, you would require on-going treatment for this. Colchicine is being used with success with increasing frequency.

Yes: You have multiple reasons for pain, since pleurisy and pericarditis can leave adhesions which can cause pain. The parenchymal scarring at the apex may be unrelated to your symptoms.
