Not related : Ketones in urine can come from anything as benign as "fasting" or spacing meals too long. Pain in left upper quadrant could be from "gas" which accumulates in the colon fold in that corner. I don't hear anything worrisome in your symptoms but check with your doctor.

Probably nothing: +1 ketones in the urine seldom means anything beyond that you were hungry. Transient twinges are very common -- i'm betting this is texidor's, a knife-like pain that is worse when you breathe in, due (i think) to a fold in the lining membrane of a body cavity.
