Patella: Did you have your knee cap replaced? That is one possible cause if it was not replaced. Did you have a work up for infection? When was the bone scan done? Bone scan can be positive for up to 18 months after replacement but only mildly positive. If the knee cap was replaced, loosening is a possibility as well.

3 phase bone scan: I'm assuming that you had a nuclear medicine 3 phase bone scan. Usually this test is done after tkr to evaluate for infection or loosening. If 3 phase bone scan is abnormal, indium scan and possibly sulfur colloid scan may need to be done to increase the specificity of the test.
