Yes: bone on bone means that the cartilage has worn away and the internal hydrolics that allow separation of bones that form the knee are gone. an off loading brace will control the symptoms by not allowing the bone on bone to occur, but if you are that symptomatic and have no serious medical problems that would threaten your health, replacement is the best technology at that stage.
