There is no way: To tell from so far away. Safest thing to do would be to return to the doctor who placed the iud. That is very reasonable considering you have moderate pain. Don't delay. Good luck!

Pelvic exam: You should have the doctor who placed it perform a pelvic exam. You could have a perforation or it could be a cyst as well.
