Cracked rib?: Point tenderness with sharp pain can easily be due to a broken rib, which will only show up if displaced or x-ray happens to go thru break. Can occur with local trauma, hard cough, etc. A good exam looking for this may find it ; should get recheck to be sure is not some other cause of bad pain, like a stone, etc.
