Yes it is rare: Ludwig's angina is hard to miss. Don't worry. If you feel you're getting worse, go to your doctor or dentist. Azithromycin is not typically used for these sorts of infections unless one is allergic to penicillin.

Rare complication: ..But it does happen. Your treating dentist should be advised that the zithromax s/he prescribed is not working if you feel the swelling is getting worse. Anxiety in and of itself will not cause ludwig's angina.
