Probably: Diseases of the blood vessels and heart are the leading cause of death in the US. Tobacco use is an important etiologic factor in both. Use of tobacco kills in many more ways than just lung cancer. Smokers have more cancers of many other organs, lung disease other than cancer and have more accidents.

Yes: Heart disease is still the leading cause of death, and smoking is a large risk factor for it. So if you are in the US and smoke you are more likely to die from CAD than lung cancer.
