No: Botox blocks the transmitting of a nerve impulse to the muscle it does not harm the muscle directly. The effects are gone in under 6 months most of the time by 4 months. It may be possible that the muscle has weakened by not contracting for a few months but the strength should return as soon as the muscle tone returns.

Botox complication: Extremely unlikely. How would you get a "permanent " muscle injury and what do you mean by "damage"? Besides, Botox wears off in 3-4 months. There is nothing permanent about Botox's affect on muscles.
