complicated: You normally have bone conduction and air conduction but the former is overshadowed by the latter. You're hearing your carotid artery which traverses the temporal bone where your inner ear is located. If you're hearing it, your air conduction is reduced. The commonest cause is serous otitis media. A good ear exam will help determine the cause.
