Pain...: Not typical for a chronic hydrocele, but can be part of an acute presentation, i.e., if this is new. Hard to say what else is going on, but some possibilities could include infection of the epididymis or a hydrocele causing enough pressure to entrap the spermatic cord in the inguinal canal ; threaten the blood flow to the testicle. I wouldn't wait long to see a urologist (again) about this.

Should see doc..: Hydrocele (sac of fluid), or more likely varicocele (collection of veins) at your age, is not a dangerous condition, especially just left side or on both. However, if it gets large and can become quite heavy on upright position and can cause pain. Because it originates from the inside, aching can radiate into the lower abdomen. I suggest getting evaluated by doc. Good luck.
