Not usually: A small amount of fluid in the cul de sac is normal. The abdominal/pelvic cavity is moist with peritoneal fluid and when standing or sitting gravity causes the fluid to pool in the cul de sac. Larger amounts can be seen with a ruptured cyst and will usually resorb on it's own.
