No doesn't have to: A small cavity can be filled.Depending on the location..Sometimes may need crown..It would only need to have root canal if nerve involved or extraction if unrestorable with fill..Or crown..The best way is to see dentist and can go over different options.

Possibly neither one: ....... If the cavity is of a small size, your dentist can most likely fix it with a simple filling. If the cavity is penetrating deeper, then you might need a root canal treatment, followed by a crown. And if the caviry is encompassing major area of your tooth that renders it un-restorable, ....Then you should think about having it removed (atraumatically), and later on implant and a new crown.
