The xray: finding was likely spurious, a chest xray is not the most accurate way to evaluate heart size unless it is grossly abnormal.  A "borderline" enlarged heart on chest xray is usually normal, which was confirmed on your echo.

Echo normal? : If your echo is really normal, i wouldn't be concerned at all. X-ray criteria for enlargement of the heart are anient and inexact while echo is highly accurate - within a millimeter or two. Go with the echo result!

Cardiac evaluation: A "family history" of lung cancer often means multiple family members smoked. At least on chest x-ray (cxr) there doesn't sound like evidence of lung cancer. However, the heart is abnormal. I'm not sure how congestive heart failure (CHF) has been ruled out. Need ct chest possibly, echocardiogram, bnp, and cardiologist evaluation. In some cases a heart biopsy is done.

Echocardiogram: An enlarged heart simply means that there is something wrong.  It should be followed up with an echocardiogram which will accurately determine the size of each of the chambers and provide useful information as to the cause of the enlargement, if that is, in fact, confirmed.

Not a reasonable fea: If two months ago you had the same symptoms and had an echo and cxr which were normal and you now have the same symptoms, you have demonstrated that your symptoms are not due to an enlarged heart.  You have very little to fear about the heart mysteriously enlarging in the interim.

Answer: the hedge.

On a pa/lateral chest xray the heart may be up to 50% of the thoracic diameter. Beyond this the heart is said to be enlarged- i.e. A sign of heart failure. If the heart barely meets this criteria or is slightly larger the radiologist may say that it is "mildly enlarged" or "heart size is borderline".

Not too bad, but: not good either. A hemoglobin of 109 suggests anemia and needs to be evaluated. Large heart and shadows in the lung may suggest heart failure and the two may be related, It would be prudent to see your doctor as all of these things could be readily treatable.
