Probably: Four years of cigarette smoking typically isn't going to cause a lot of damage to your lungs or anyone else's. The problem is that four can quickly become 40 and then you'll really be sick. : Debris from the cigarette smoke ends up in your lung and remains unless its self-cleaning system can remove it. You did not remove all benefits of quitting for those years, but you are well on your way to that outcome. If you can avoid them from now on your lungs will improve weekly and within 6 months, you may be where you were.

Never too late..: Congratulations! your blood pressure will start to return towards normal within 15 minutes, your heart rate will respond within days. Your lung function can take longer to improve, it has to heal, and some of the changes may be permanent, but at your age hopefully mild. Your risk for lung cancer drops to that of the general population somewhere between 5-10 years after quitting. Hang in there!

: No, but it will take 6 months for your lungs to remove the residual debris left from the new round of smoking. At that point you are likely back where you were if you stay away from smoking.

No but it's good: You've cut your heart attack risk significantly but by no means altogether. The increased cancer risk from smoking lasts for maybe two decades, but again you've reduced it. More important, the fact you stopped smoking guarantees breathing will be easier when you're older and you're more likely to be able to do the things you enjoy. Again, glad you quit.

Smoking cessation: Difficult problem unless the patient wants to quit. It is behavior that creates tobacco addiction, like addiction to any other substance. And for that behavior to change it requires motivation by the person addicted. There are support groups who in addition to a working with a behavioral professional can assist with some medications to help smoking cessation.

Keep at it: Addictions are chronic relapsing diseases and one has to keep at it.  Typically it takes 6-7 tries.  Give yourself at least six month with the patch and if you are highly dependent on nicotine based on serum cotinine levels you might benefit from a patch for the rest of your life.  It's tobacco not the nicotine which is dangerous.  At www.drrotcford.com under handouts  there's a handout.

Nicotine dependence: Is considered more difficult to kick than narcotic dependence.  Try nictotine gum or patches, then you need group support.  Acupuncture is said to help.  Good luck.
