Cyst: Without having seen the images, I can only talk in general.  White areas on MRI are commonly cysts or fatty tumors.  These are benign and not to worry about.  Occasionally, a white area can be a cancer or infection but this is easily differentiated from a cyst or lipoma.  Your doctor should be able to tell the difference from the pictures.

Brain size : The human brain reaches its maximum size around age 20-22, then it slowly shrinks over time.  By age 75, it may decrease in volume or mass about 30%.  This is a natural phenomenon.  It is not clear what the biological purpose of this is.

The mass of the brain also decreases after injuries, such as traumatic injury, stroke, MS, or illnesses, such as certain infections, brain tumors, or dementia.

MRI doesn't explain: "No mass, no lesion detected" in a Radiology report usually means a normal brain MRI scan. I assume that the scan was ordered for a reason (i.e., symptoms). There are many symptoms (I would say MOST symptoms) that prompt an MRI scan &amp; cannot be explained by it. MRI looks for ANATOMIC problems - tumors, bleeding, etc. The problem here is FUNCTIONAL - the nerves function incorrectly. TTYD who orderd

Neurosurgeon: I believe that a neurosurgeon is best qualified to answer your questions after an assessment of your symptoms, if any, &amp; examination  and after reviewing your MRI images.

how big would a tumor have to be to cause a midline shift or mass effect in the brain? is this what rads. look for on cts or the actual tumor?

Look for everything: Depending on the clinical presentation, you would generate some possibilities- you would have to evaluate everything- is there any bleeding, is there any signs of ischemia, is there any mass ( if the contrast is used), if there is a mass- where is the location, how big it is, is there any mass effect, edema, bleed around it etc. Remember head is a close space-any mass can increase pressure inside.

Depends: Seizures are uncontrolled electrical activity in the brain.  If you were told you had stress-induced seizures, they may be pseudoseizures or non-electrical seizures which are difficult to treat.  Brain masses can be benign, but without knowing exactly what kind of mass and where it is i can't state whether it will ever be dangerous or not.  Your doctor should be able to answer the question.

Accepted Answer

Yes: You should consult with both a neurologist and a neurosurgeon. Chances are you are going to need prophylactic seizure medications and another course of TB therapy. You may need surgery (depends on location and response to meds) Be sure to monitor seizure meds, Vit D level and folate (folic acid) while on therapy.

can a 10 mm arachnoid cyst even press on the brain?? is this correct?

Hard to tell: It is not feasible to evaluate without reviewing films.  It is even more challenging to speculate whether the cyst compresses brain tissue.  Best to have a radiologist review the images with you and point out whether such impacts are present.

: No, it does not mean that. Your neurologist will discuss the result in detail, good luck.

Sleep?: If you have poor sleep with excessive daytime sleepiness you could have a sleep disordered breathing problem.   This can affect overall health including immune system, hormones, focus, concentration, memory, etc. If sleep is a problem  get a sleep study done to see how you are sleeping.  You may also need to see your md to get blood work up done to check for any disease processes that are going on.
