When needed: I'm not sure what you're asking. In terms of best time of day these can be taken at any time, though I might not take either too close to bedtime (say, within 2 hours of bedtime).

Stomach Acid: Stomach acid is pretty strong, much stronger than vinegar. If your stomach acid does not kill the bacteria in the probiotics, the vinegar won't either.

Stop using it: I am glad you realize that the claims for this stuff are mostly lies. Let your skin heal on its own -- it'll take a few months but it will -- and consider yourself fortunate not to have been harmed further by quack claims. Yes: Apple cider vinegar should not interfere with absorption of iron supplements. Taking Vitamin C with iron improves absorption.

Genital wart Rx: It's best not to self treat without seeing a doctor first. No over-the-counter treatments have been studied for their benefits for genital warts, and none are as reliable as prescription medications or treatments provided in the clinic (e.g. freezing, laser cautery). Vinegar of any kind is totally ineffective but this or OTC treatments might make diagnosis harder when you see someone. Don't do it.

Caution: The answer depends on how you want to use apple cider vinegar. If you use it as a salad dressing, you should be fine. But taken as a daily medical treatment, it could be a little more risky. Yes, some studies of apple cider vinegar are intriguing. But a lot more research needs to be done. Right now, there is not enough evidence that apple cider vinegar -- or any vinegar -- has any health benefit.

In general, yes: Each person reacts differently to any therapy so the bottom line is what works for you.  Having said this, a good caveat is always take the least amount of anything for the optimum result.  So, try decreasing the amount of apple cider vinegar to find the lowest dosage that will control the acid reflux.  If adequate control requires 2 tablespoons 3 times a day, so be it.

It is unlikely that: these natural products will harm you. Grapefruit extract can interfere with other medicines you might be taking but on its own is OK. Vitamin D can be toxic at extremely high levels. Limiting your intake to less than 2500 IU/day is advisable. Ultimately the acidic content of vinegar will put a workload on your kidneys but in reasonable quantity is probably OK. Ginger should be OK.
