When prescribing for pts on dialysis, it is essential to consult a reference guide to determine if the drug is subject to renal clearance and requires a dose adjustment. Best to discuss with a nephrologist.

Safe pain meds: The safest is tylenol (acetaminophen). Narcotics used properly are safe.  Nsaids like Motrin allege and Aspirin are risky.

Dialysis: If the  medication is eliminated by a normally functioning kidney, it can accumulate to dangerous levels in between dialysis treatments.  Its best to check with your nephrologist (kidney doctor) before starting a new medication.

is it possible for a chronic kidney stage 5 patient to prolong dialysis and kidney transplant by just specialised medication and strict diet?

Yes: Yes; an appropriate (but relatively strict) diet would have to be adhered to as recommended by your nephrologist.  Exercise and hydration are equally important. Stage v renal disease, however is near " end-stage" and will likely progress into renal failure at some time.  Peritoneal dialysis is an extremely appropriate, and oftentimes under used/recommended option, especially for younger patients l.

Yes, needs dialysis: Immunosuppression is meant to prevent rejection of the transplanted kidney, graft rejection is a separate disease apart from the function of the new kidney. This is a complex issue, nephrology assessment and follow up is needed, please seek their input and care, good luck

i am covid 19 positive with no symptoms other than my blood pressure that is very high, regardless of doubling my normal medication. i am on peritoneal dialysis.

COVID 19:  We don't know how high your baseline blood pressure is or what it is now.  But if your blood pressure is very high- definitely recommend that you contact your physican at minimum. If it is really elevated you may need emergency care to help reduce it. Extremely high blood pressures can become life threatening.

Treatment of CKD: At CKD stage 4, you do not need dialysis at present.  If you CKD progresses to end stage renal disease, you will need to start on dialysis or have a transplant. You do not list that you have diabetes or hypertension, if the cause of your CKD is polycystic kidney disease, not much can be done for your decline into higher stages of ckd. Your nephrologist is the one to speak to about your prognosis.

patient x has one kidney and a creatine level of 2.85. he doesnt drink a lot of water. is is on bp meds to lower his bp and i believe cholesterol medication. what should he do to prevent having to go on dialysis or ckd?

: He should stay hydrated. He should ask his doctor if an ACE-inhibitor would be helpful. He should see a nutritionist and get on the correct diet to avoid high potassium and magnesium levels, etc. He should avoid medications that can harm the kidneys-his doctor can provide those to him. hi doctors. my egfr has ranged from 59 to 57 to 50. i am frightened about possibly having to go on dialysis in the future. can i do or get any medications or medical treatment with this range of chronic kidney disease-ckd to possibly delay this?

: Yes, the first is to find out the cause of your decline in eGFR.  Then treat the cause. There are hundreds of causes.  If diabetes and/or blood pressure above 120 getting it lower may slow progress.

We used to plot 1/creatinine on the y axis and time in months or years on the x axis.  The slope of that line will give you some idea of when you might need dialysis.  So find every eGFR and plot.
