Fungus: Histo is a very common fungus.
Histoplasmosis is the disease state, and includes in the lungs, liver, spleen with small and potential enlarging non cancerous nodules.These can be of a size to be seen on ct. 
Larger than 3 -5 mm. And are frequently multiple in an individual.
Most  are asymptomatic and found on x-rays and cts.

fell on my back , had cat scan , revealed (2 ) 4mm lung nodule pulmonary  doctor says i should have nothing  to worry about, follow cat scan in 6 mnth?

Agree: Lung nodules that small, in someone your age are probably nothing to worry about.  I agree that a follow up scan is necessary to ensure that they do not enlarge.

radiologist saw 3mm lung nodule. he mentioned it but never mentioned it again in the impression part of the report. do i need follow up? im a non smoker. they did ct of abdominal and saw this. if i have one could the rest of my lungs be full of them?

: The scan included an area of your lung with a small nodule. The characteristics of the nodule  is important information. It is possible to have multiple nodules, if the scan included only a partial view. Do you have current symptoms? A small nodule without significant medical risk factors without involvement of the lymph nodes or other areas of the lung may be followed. TB test may be indicated.

worried about a solid lung nodule 4 mm in 2009 and is now 7 mm, non calcified, smooth borders.  rad said it looks like benign soft tissue, possible granuloma.  do non calcified granulomas grow, and could a cancer grow at that growth rate?

Yes and yes.: Get an opinion from pulmonologist or thoracic surgeon regarding opinion about possible biopsy.  It depends whether or not the benefits of biopsy outweigh its risks.

Lung nodule: Lung nodules are densities in the lungs seen in imaging procedures. The indication depends on how they look like, calcified or not, cavitary or not. They can be scars, old granuloma, localized infection (benign ones) or they can be cancers. That is why when we see them we are aggressive in working them up to make sure they are diagnosed early as if cancer, best treatment is removal if not spread.

Lung nodule: Short term follow up is recommended ( every 6 months) for up to 2 years. If it increases in size then a pet scan and possible biopsy can be performed.

Inflamation, etc: Benign nodules are often the result of abnormal tissue, inflammation and/or scar. This can result from inhaling something or having an infection.

Lung cancer: It would take longer than a few years of smoking to get lung cancer.  But you can get cancer at any age and sometimes the cancer will spread into the lungs.

Depends: Depends on what the nodule is? First you need appropriate diagnosis.  This is best by a multidiscipline comprehensive thoracic surgical lung nodule program. If the nodule is a tumor, treatment will depend on type and staging if malignant.
